I/We declare that by bringing stock to the market we agree to comply with the following conditions:
(i)The vehicle transporting livestock into the market as above is within our own ownership and was cleansed and disinfected prior to loading these animals or
The vehicle trnsporting livestock into the market as above is not in our ownership and (for journeys of over 10 kilometers) has been cleansed and disinfected.
(ii) No susceptible animals have moved onto the holding within the last 6 days.
(iii) That we will observe all procedures outlined in the Standard Operating Procedure within the market as displayed in the market office.
(iv) We have examined the stock and have seen no signs of FMD or other notifiable disease.
(v) I/We declare that the details given above are correct for the animals listed thereon.
(vi) All animals born after 01/07/1996 are accompanied by the correct passport in accordance to MAFF regulations.
(vii) All animals born after 01/01/1998 are double tagged in accordance to MAFF regulations.
(viii) The above Farm Assurance details are correct.
